PacifiCare Signature FreedomSM
With the PacifiCare SignatureFreedomSM
(SDHP) plan, you can enjoy the freedom
and flexibility of a traditional
PPO plan with the added feature
of Self Directed Account (SDA).
The SDA is fully insured by PacifiCare

$1,000 for certain routine and preventive

Any unused funds carry over year
after year, for as long as you are
enrolled on the plan.
Once you deplete your SDA, or if

a PPO plan.
You must meet annual deductibles.
Eligible medical services paid from
the SDA will count toward satisfying
the PPO deductible.
